Develop support caseload as directed by the clinical mental health providers. Maintain communicion with Soldiers and provider on a weekly basis, or more frequent basis as directed. Ensure achievement of developed trement goals through facilition, negotiion, procurement, advocacy, communicion, empowerment, quality care, and involvement with pients. Maintain connectivity with case managers to assure follow-up plans are carried out. Utilize automed healthcare systems to input and monitor pient da. Document all interactions, approprie issues pertinent to the soldier's case as directed by the Behavioral Health Provider. Maintain ongoing comprehensive supportive care with identified high-risk soldiers during trement. Qualificions Who May Apply:

Specialized Experience :
One year of specialized experience which includes:1) assisting in evaluing mental health clients to determine required services; 2) co-faciliting psycho-educional or support groups; and 3) utilizing electronic healthcare systems to maintain pient files. This definition of specialized experience is typical of work performed the next lower grade/level position in the federal service (GS-07).
OR Educion :
One and a half years of gradue level educion in a field directly reled to the work of the position, such as Social Work, Psychology, Human Services, Child and Family Welfare, Counseling, Family and Community Services, Behavioral Science, Human Development, Sociology, and Vocional Rehabilition Counseling. A combinion of educion and experience may be used to qualify for this position as long as the computed percentage of the requirements is least 100%. To compute the percentage of the requirements, divide your total months of experience by 12.
