Provide case management. Conduct bi-annual and quarterly assessments. Individual and group counseling to elderly participants. Documentation in progress notes of provided case management services. File in ptps' charts if relevant for future references. Conduct Home Evaluations as needed. Other office work as assigned. Must be bilingual in Chinese (Mandarin and/or Cantonese) . Rotating Saturdays.
