This role includes accompanying consumers to medical appointments, assisting with daily living activities, and supporting nursing department operations through administrative and care-related tasks.

Friday Job Responsibilities:
- Medical Appointment Coordination
- Safely transport consumers to scheduled medical appointments as directed by the Nursing Supervisor.
- Provide accurate and relevant health-related information to healthcare providers, ensuring the continuity of care.
- Effectively communicate appointment outcomes, including diagnoses, treatment plans, and follow-up requirements, to the assigned LVN or RN.
- Maintain thorough and timely documentation of appointment details in the consumer's medical records in accordance with organizational protocols.
- Direct Consumer Support and Care
- Assist consumers with activities of daily living, including grooming, bathing, toileting, and personal hygiene, while promoting independence and dignity.
- Facilitate self-care and independent living skills training in areas such as personal hygiene, health management, and nutritional practices.
- Provide specialized training to enhance social-emotional development, focusing on self-awareness, effective communication, behavioral management, and relationship building.
- Support consumers' mobility and physical health by assisting with walking, stretching, and other exercises as needed.
- Perform specialized tasks, including the collection of laboratory specimens, under the guidance of nursing personnel.
- Administrative and Nursing Department Support
- Collaborate with nursing staff to ensure comprehensive, high-quality medical care for all consumers.
- Deliver clear, concise medical instructions to facility supervisors and other relevant staff.
- Monitor and manage medical supply inventory, ensuring availability and proper stocking of essential items.
- Coordinate nursing department activities, including organizing schedules, maintaining records, and supporting office functions.
- Adhere to established safety, environmental, and infection control protocols to maintain a secure and sanitary environment.
- Participate in miscellaneous job-related duties as assigned by supervisory staff.
- Uphold professional appearance and comportment in compliance with TLC's Dress Code policy.
- Attend and actively participate in mandatory in-service training and staff meetings to maintain up-to-date knowledge and skills.
- Other duties as assigned by supervisor.
Qualifications:
- High school diploma or equivalent educational attainment.
- A minimum of one year of experience in a healthcare or human services environment, preferably working with individuals with developmental disabilities. Preferred.
- Proficiency in documenting care activities, with a focus on accuracy, clarity, and timeliness.
- Strong punctuality, reliability, and time-management skills.
- Knowledge of standard patient care practices, medical terminology, and proper charting techniques.
- Professional demeanor with excellent verbal and written communication skills.
- Ability to build positive working relationships with colleagues, consumers, and external stakeholders.
- Team-oriented mindset with a collaborative, proactive approach to problem-solving.
- Valid California driver's license with at least three years of driving experience and a driving record compliant with TLC's insurance guidelines.
- Physical ability to lift and transfer weights ranging from 50 to 100 pounds.
- Successful completion of all required background checks.
PHYSICAL DEMANDS AND WORKING CONDITIONS
This position is primarily housed in an open air-conditioned office. Daily, there are outside activities and therefore the working conditions are subject to weather elements, i.e. sun, wind, rain. Primary duty will be driving consumers from all TLC homes or Day Programs, as required, to doctor's appointment. This position is moderately physical. There is frequent standing, bending, sitting for periods of time, and fast-paced walking. Assisting consumers in wheelchairs, with walkers, gait-belts that require pushing, supporting and guiding. Moderate lifting of weight that varies between 50-100 pounds. Due to frequent driving the working conditions are subject to weather elements, i.e. sun, wind, rain. On occasion, consumers may become physical including but not limited to hitting, kicking, grabbing, and biting. Work environment involves exposure to potentially dangerous materials that require following safety precautions and may include the use of protective equipment. Frequently, you are exposed to consumers' bodily fluids which can include saliva (drool), blood, urine, fecal matter. Stringent Universal Health Precautions are practiced.
