- Be at the forefront of discharge planning, mental health coordination, and resident support services.
Key ResponsibilitiesDaily Responsibilities:
Participate in stand-up and stand-down meetings to discuss resident needs. Conduct resident interviews and complete MDS sections (C, D, E, Q). Document social work quarterly notes in PCC. Follow up on resident grievances and dignity audits. Assess social history, social determinants of health , and psychosocial well-being. Lead discharge planning and care plan development for psychosocial and mental health needs.Weekly & Monthly Responsibilities:
Coordinate visits with therapists and social workers from local mental health authorities. Organize and lead care conferences with residents and their representatives. Conduct POLST audits, psychotropic medication reviews, and QAPI meetings. Oversee resident council meetings (if designated). Assist with scheduling vision, podiatry, and dental visits.As Needed:
Provide staff training on social work-related topics. Issue NOMNCs/ABNs for Medicare-certified facilities. Work with LCSWs to support residents' mental health needs. Review and process PASRR forms for new admissions and significant changes.What We're Looking For:

- Ability to manage multiple responsibilities effectively. ✔ Strong Communication Skills
- Ability to collaborate with residents, families, and healthcare teams. ✔ Experience in Long-Term Care (Preferred)
- Familiarity with MDS documentation, PASRR, and care planning. ✔ Degree in Social Work or Related Field (Preferred)
- Candidates with relevant experience are encouraged to apply.
