Job Description

At AllHealth Network , compassion, connection, and lived experience come together to support individuals and families on their path to recovery. The Forensic Assertive Community Treatment (FACT) program is an evidence‑based, interdisciplinary team model serving individuals with severe mental illness, often with co‑occurring substance use disorders and justice‑system involvement. FACT provides intensive, community‑based services focused on promoting stability, recovery, and successful community integration through wraparound care coordination, psychiatric services, substance use treatment, peer support, and collaboration with judicial and community partners. II - FACT
, you will be an essential member of this multidisciplinary team, using your lived experience to build authentic, supportive relationships that promote hope, self‑determination, and long‑term recovery in community and field‑based settings. Build safe, supportive peer relationships that foster trust and open discussion of recovery goals and options. Support the recovery path chosen by each individual, including but not limited to self‑help groups, harm reduction, medication‑assisted treatment, abstinence‑based recovery, hospitalization, psychotherapeutic interventions, spiritual supports, or independent recovery. Provide peer mentoring, recovery coaching, and skill modeling to support coping strategies, self‑advocacy, and community stability. Facilitate and co‑facilitate recovery‑oriented groups and activities that promote connection, empowerment, and skill‑ Conduct telephonic, virtual, and face‑to‑face outreach and engagement in community, court, and field‑based settings. Assist with care coordination tasks including appointment support, transportation coordination, system navigation, and communication with providers and care team members. Collaborate closely with clinicians, nurses, psychiatrists, care coordinators, probation/parole officers, and community partners to ensure integrated service delivery. Maintain a small caseload of individuals requiring ongoing peer recovery support to promote stabilization and reduced system involvement. Accurately document services in the electronic health record and contribute to data collection supporting billable services and program outcomes. Participate in daily team meetings, supervision, trainings, and crisis response as needed to support cohesive FACT team operations. Perform other duties as assigned within the scope of the role and program needs. High school diploma/GED certification. Prefer completion of formal peer specialist training in specific content areas as outlined by the state of Colorado. Minimum of one year of active, engaged personal recovery from Substance Use Disorder and/or Mental Illness. Knowledge of multiple paths to recovery including therapeutic interventions, medication assisted treatment and different community-based self-help options. Possess excellent customer service skills, strong organization skills, flexibility in responding to multiple demands, able to complete routine work with minimal direction from supervisor. Communicate effectively with health care team, clients and families, and community providers. A culture rooted in compassion, care, and integrity, where clinicians are valued as people first.
