Job Description

Job description Riverbend Health Care Center is a skilled nursing/long term care facility in Loveland, CO. We are currently seeking a welcoming and approachable individual to join our family as a social service assistant.
Job summary:
Responsible for accurately assessing patients for admission, including: clinical needs, appropriateness for admission to SNF/ Sub Acute program(s), and assuring adequacy of financial support prior to admission. Facilitates interdisciplinary plans in collaboration with interdisciplinary team(s), and assures Utilization review reports are completed and provided to payor as needed. Serves as a liaison between patient, physician, care team, payor, and the discharge planner by coordinating, monitoring and communicating patient's progress and cost evaluation while in the facility. Assists with coordination to the next care level. Assists Executive Director and Managed Care Resource with census management.
Responsibilities:
Admissions process 1. Assesses prospective patients to evaluate care needs and determine suitability for admission to the program(s); contacts payor sources to determine or verify coverage and funding for required patient services and length of stay. 2. Determines reimbursement based on patient clinical needs and payor contracts. 3. Communicates information to care team and coordinates patient's transition from admission through discharge. 4. Attends facilities weekly UR meetings; collaborates with interdisciplinary team to coordinate patient care while in SNF. 5. Obtains accurate information from physicians, patient, and payor source regarding the expected discharge plan and communicates information to the interdisciplinary team(s).
