Conduct client intakes and assessments to identify social, medical, and behavioral health needs, establishing a foundation for individualized service planning. Assist in developing and implementing service plans in collaboration with clients, families, and supervising professionals, ensuring goals reflect client needs and available resources. Coordinate and monitor services across healthcare, housing, education, and social support systems to promote continuity and effectiveness of care. Perform home and community visits to evaluate client progress, provide ongoing support, and adjust service activities as needed. Serve as an advocate and liaison between clients and community agencies, helping clients access benefits, entitlements, and other supportive resources. Maintain accurate, timely documentation of all client contacts, interventions, and service outcomes in accordance with agency and confidentiality standards (HIPAA). Prepare progress updates and supporting reportsincluding summaries for supervisors or court requirements when applicable. Collaborate and communicate with interdisciplinary professionals including physicians, psychiatrists and social workers to ensure coordinated client care. Participate in supervision, team meetings, and professional development to maintain service quality and comply with agency productivity and reporting expectations.
