$23-$26/hour Join Our Compassionate Care Team! West Delray Nursing and Rehabilitation Center is seeking a dedicated and compassionate Social Services Assistant to support the emotional, psychosocial, and discharge planning needs of our residents. In this role, you will work closely with residents, families, and the interdisciplinary team to ensure each resident receives the support, resources, and advocacy needed to enhance their quality of life. Responsibilities Assist in implementing the facility's social services programs under the direction of the Administrator and Director of Social Services Assess residents' social service needs and participate in facility assessments Coordinate with community agencies and resources to support resident care and discharge planning Collaborate with the interdisciplinary team (IDT) to address residents' psychosocial and emotional needs Assist residents and families with financial assistance applications, referrals, and community resources Support behavioral health initiatives and resident-centered care planning Participate in resident and family council meetings as requested Educate residents and families on resident rights, advance directives, and available services Complete social services documentation including MDS, BIMS, and cognitive assessments Coordinate resident transfers, discharges, and appeal processes when applicable Assist residents and families in selecting appropriate post-discharge care providers Maintain knowledge of current federal and state regulations related to social services in long-term care Support residents with limited English proficiency (LEP) through translation resources and communication assistance Participate in facility surveys and interdisciplinary meetings as needed Qualifications Minimum 1 year of Nursing Home, Long-Term Care (LTC), or Skilled Nursing Facility (SNF) experience required Experience with Medicare, Medicaid, and community-based resources preferred Excellent communication and interpersonal skills Strong organizational skills and attention to detail Ability to multitask and meet deadlines in a fast-paced environment Compassionate, professional, and resident-focused approach to care Benefits Comprehensive Medical & Dental Coverage Generous Paid Time Off (PTO) 401(k) Retirement Plan Employee Recognition Program Supportive, team-oriented work environment Company perks and employee discounts Apply Today!
