$20 per hour Join Our Caring Team! Breezy Hills Rehabilitation and Care Center is seeking a motivated and compassionate Social Services Assistant to support our residents, families, and care team. In this role, you'll help create a positive and supportive environment, ensuring the emotional, social, and psychological needs of our residents are met.

Key Responsibilities:

Assist with the daily operations of the Social Services Department in compliance with federal, state, and facility regulations. Support discharge planning, care plan development, and resident assessments . Collaborate with administration, nursing, and medical staff to address resident needs. Encourage resident and family participation in care plans and meetings. Promote Residents' Rights , dignity, and respect among staff and residents. Follow infection control protocols and report exposures promptly. Participate in safety and emergency preparedness training. Maintain CPR/First Aid certification and complete required ongoing training.

Qualifications:

High school diploma or equivalent required. Minimum 1 year of experience in a nursing home or healthcare setting preferred (but not required). Experience in an office setting with strong administrative and organizational skills . Strong communication skills in English (both verbal and written). Empathy, compassion, and professionalism when working with elderly residents and their families. Ability to stay calm under pressure and work effectively as part of a team. Basic computer skills.
