9:00am-5:30pm Miami, FL Full-Time AIDS Grants Summary The South Florida AIDS Network/Outreach workers must work with service providers, including medical case managers, to locate people lost to medical care or medical case management and bring them back to care. The medical case manager, or pharmacy staff, after repeated attempts to contact the client by phone and mail without success, may refer the case through a system certified referral to an outreach worker. Responsibilities Staff will accompany newly diagnosed, lost to care, or otherwise unconnected HIV+ clients to the initial physician appointment, medical case management appointment, and/or intake at a residential substance abuse facility of the client's choice for the purpose of reconnecting them to care or enrolling them in service. Make home visits to meet with a client for the purpose of connecting them to care. Conduct brief intakes for new clients and enter data into the system outreach registration screen. Act on approved referrals, review data in the system for existing clients who are lost to care or are at risk of falling out of care. Assist clients, as necessary, for the purpose of helping them to obtain necessary documents for entry into the service system. Obtain all required consents for the outreach worker to access client-related information in the Ryan White Program. Provide education on available care and treatment options and services available to HIV+ individuals with the goal of directly empowering and enabling the client to access existingHIV/AIDS
service programs, including Counseling & Testing sites. Provide out-stationed linkage and coordination to care services at key points of entry, including but not limited to counseling and testing facilities and other facilities with a high percentage of HIV+ clients as identified by the counseling and testing facility and verified by the Ryan White Part A Program. Coordinate and participate in planned outreach/testing events in cooperation with the Miami-Dade County Health Department. Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise). Performs all other related job duties as assigned. Experience Generally requires 0 to 3 years of related experience. Education High School diploma is required. Skill Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines. Ability to communicate effectively in both oral and written form. Ability to handle difficult and stressful situations with professional composure. Ability to understand and follow instructions. Ability to exercise sound and independent judgment. Knowledge and skill in use of job appropriate technology and software applications. Credentials All staff providing Peer services must be certified through the State of Florida's Department of HealthHIV/AIDS 104
course. Unit Specific Credential Working Conditions Physical Requirements - Job function is sedentary in nature and requires sitting for extended periods of time. Function may require frequent standing or walking. Must be able to lift or carry objects weighing up to 20 pounds.
