PM Salary:
$23-$25/hour Sun Harbor is seeking a compassionate and organized Social Services Assistant to support the Social Services Department in delivering high-quality psychosocial care and resident support within our skilled nursing and long-term care community. This role plays a vital part in resident advocacy, discharge planning, care coordination, and ensuring residents and families receive the support and resources they need throughout their stay. Position Summary The Social Services Assistant supports the planning, coordination, and implementation of comprehensive social services programs for residents and families. This role collaborates closely with interdisciplinary teams to promote resident well-being, safe transitions of care, and compliance with facility policies and regulatory standards. Key Responsibilities Assist in planning, organizing, implementing, and evaluating the facility's social services programs Support the development and coordination of Social Services Department policies and procedures Identify residents' medically related social, emotional, and environmental needs Participate in resident assessments, care planning, and discharge planning processes Interview residents and families to obtain social histories and assess needs Encourage resident and family involvement in care planning and social service programs Coordinate discharge arrangements, transportation, and community resource referrals Provide guidance regarding Medicare, Medicaid, and financial assistance programs Collaborate with nursing staff, physicians, and interdisciplinary teams to ensure effective care coordination Maintain accurate, timely, and detailed documentation and progress notes Ensure social services sections of the MDS are completed appropriately and on time Participate in facility surveys, audits, committee meetings, and utilization reviews Assist residents and families with social, emotional, financial, and placement-related concerns Maintain positive working relationships with community agencies and healthcare organizations Assist with departmental responsibilities in the absence of the Director of Social Services Qualifications Minimum 1 year of experience in a Nursing Home, Skilled Nursing Facility (SNF), or Long-Term Care (LTC) setting required Experience with Medicare, Medicaid, and community-based resources preferred Knowledge of resident social and emotional needs in a healthcare environment Proficiency with Microsoft Office Suite and PCC Strong communication, interpersonal, and organizational skills Ability to multitask and meet deadlines in a fast-paced environment Ability to exercise sound judgment and maintain professionalism under pressure Team-oriented with a compassionate and resident-focused approach Benefits Comprehensive Medical & Dental Coverage Generous Paid Time Off (PTO) 401(k) Retirement Plan Employee Recognition Program Supportive, team-oriented work environment Company perks and employee discounts Sun Harbor is committed to creating a supportive and inclusive workplace where team members are valued and empowered to make a meaningful impact in the lives of residents and families.
