Job Description

You will work as part of a multidisciplinary team to deliver intensive treatment, support, and skill building to help stabilize youth in their home and community settings. Deliver intensive in-home and community-based services 3-5 times per week, as clinically indicatedTeach and coach youth in coping skills, self-care, and strategies for managing emotionsSupport families in addressing the emotional and mental health needs of youthCollaborate in the development and implementation of individualized treatment plansProvide behavioral coaching to enhance problem-solving, communication, and family dynamicsFacilitate linkages to community resources and make referrals through warm hand-offsCo-facilitate team meetings and assist with aftercare planning Who You Are Associate's or Bachelor's degree in Human Services with relevant experience, or enrolled in a Master's degree programExperience providing therapeutic support to youth and families preferredKnowledge of mental illness diagnoses, behavioral modification techniques, and coping strategiesFamiliarity with child-serving systems and community resourcesStrong communication, time management, and organizational skillsAbility to work with diverse populations and demonstrate cultural competenceValid driver's license and reliable transportation required We Hire for Purpose!
