CSW LSW Social Work Assistant

Responsibilities:

Identifies cognitive impairments, mood problems and other psycho-social needs in residents. Maintain accurate, thorough, and timely documentation. Review Advanced Directives and provide education to residents and family members as needed. Help provide a smooth admission and discharge process for residents and family members. Other duties as assigned by Director of SW or facility Administrator

Qualifications:

Prior experience managing staff is preferred. Ability to work independently or part of a group. Computer knowledge. Previous long term care experience is preferred but not required

License:

Must have active Certificate/Licensure for Social Work CSW LSW Complete Care at Corsica Hillsis an equal opportunity employer.
