PM Summary:
The Recovery Coach works with our Addiction Care treatment team to provide a seamless continuum of excellent care by maintaining regular and constant communication with patients to provide a set of non-clinical activities that engage, educate, and offer support to patients. The Recovery Coach will possess characteristics of compassion, active listening skills, evaluative skills, reporting skills and general communication skills to aid in helping patients to address personal and environmental obstacles to health care access and help them obtain and remain in recovery.
Responsibilities:
+ Co-facilitate peer lead support groups + Serve as a mentor for patients by demonstrating ways to become empowered and self-responsible + Possesses working knowledge of recovery including 12 step meetings, mental health, and other co-occurring disorders + Providing coaching and care coordination services for patients in within the clinic to develop patient's courage, strengths, resilience, and autonomy. + Using excellent communication skills, coordinate care and serve as patient advocate when needed + May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment from induction through discharge as needed + Work within recovery communities and other agencies to foster relationships, link people in need to resources and programs + Perform other duties as assigned
Other Information:
+ Must have completed the 5-day, 30-hour intensive Connecticut Community for Addiction Recovery ( CCAR ) Recovery Coach training. + Must be an individual in long-term recovery or a recovery ally. + 1-2 years of experience working in behavioral health or substance use disorder recovery setting preferred. + Experience working with Substance Abuse Population in an opioid treatment program, opioid health home or a primary care setting treating this disorder is preferred.
