Job Description
POSITION SUMMARY
The Social Services Assistant provides support to the HomeCare & Hospice social services team. This role works closely with social work staff and other interdisciplinary team members to support bereavement activities, volunteer coordination, communication workflows, and general office operations.DUTIES & RESPONSIBILITIES
Provide administrative and clerical support to the HomeCare & Hospice social services team. Assist with bereavement program tasks, including mailings, phone calls, tracking, and documentation support. Assist with volunteer coordination activities, including communication, scheduling support, and recordkeeping as assigned. Support established department workflows and assist with routine follow-up tasks. Recognize and report concerns, questions, or urgent issues to the appropriate team member or supervisor. Perform general office support duties such as answering phones, sending faxes, scanning, filing, and routing information appropriately. Assist with scheduling, and other non-clinical support tasks as assigned. Maintain organized records and support compliance with agency policies and departmental procedures. Work collaboratively with the Clinical Manager, social services team, and interdisciplinary team. Participate in department meetings, training, and orientation as assigned. Perform other related duties as assigned.QUALIFICATIONS
High school diploma or equivalent required; associate degree preferred. Previous administrative, clerical, customer service, or healthcare support experience preferred. Strong organizational, communication, and interpersonal skills. Ability to manage multiple tasks and maintain confidentiality. Basic computer skills required, including Microsoft Office; Excel experience preferred. Ability to communicate with patients, families, staff, and community members in a professional and compassionate manner.
