Job Description
Job Overview Ebenezer is seeking a Social Service Designee to support our adult residents in our Skilled Nursing Facility in Lake City, MN! This role assists the Social Service department in meeting the needs of residents and families through various tasks and actvities all completed under the supervision of the Social Service Director. The schedule includes; 40 hours in two weeks Day shift Monday - Friday Lake City Care Center is nestled near the picturesque Lake Pepin in southeast Minnesota. Responsibilities:
Assists with coordination of adminssions as needed and delegated byDirector of Social Services:
Provides pre-adminssion counseling, information, referral, and tours Facilitates the intake process Facilitates PAS screening and Level II Coordinates admission process with resident and resident representative, community providers and staff Assures completion of admission paperwork Informs residents of financial responsibilities, payment options and agency contacts according to the federal regulations Builds and maintains relationships with referral sources; and assists with marketing and outreach Coordinates room change process Verifies necessary payer source(s) Tracks payor renewal dates Assists with completion of payor paperwork renewals and new applications as necessary and/or follows up with appropriate resident representative to ensure necessary paperwork is completed Acts as facility liaison with counties about payor renewals and applications Assists to gather and send necessary documents to the county Follows-up with discharged patients who are/were Medicaid Pending to ensure necessary steps are followed so facility can collect/back-bill insurance Assists with paperwork for guardianship, conservatorship, and/or rep payee Assists Social Work staff with caseload assignments: Coordinates and facilitates care conferance Completes necessary social history and Social Service forms and documentation Assesses psychosocial changes and identifies strategies to address those changes Maintains records and documents as indicated and required Completes MDS and associated assessments per regulations May assist resident and resident's representative in changing primary care providers Advocates for resident rights and needs Assists with Advance Directive planning conversations and documentation Assists with investigations and follow up on grievances Assists with one-to-one and group counseling as appropriate: Coordinates Resident and Family Council Advocates for residents Provides information and referral to community service providers Assists with discharge planning as appropriate: Facilitates discharge plans for resident Arranges in home and community-based services Ensures that residents and their representatives are informed of the IDT's recommendations for discharge Provides referral to other agencies as needed Assists in Vulnerable Adult investifations and completes necessary documentation and reports as appropriate: Completes corresponding reports as appropriate (Missing item, OHFC, MARC, Police Report) All other duties as assignedRequired:
