Support Coordinator Supervisor Job Requirements:
Bachelor's degree from an accredited college or university Light lifting and extensive sitting required Some travel required Basic Computer Skills/Ability to learn and utilize new technologies Ongoing demonstration of Missouri Support Coordinator Core Competencies (see attached)
Job Purpose:
Manage a diverse caseload of individuals with disabilities, to assist those individuals in accessing funding, programs, and resources, with the goals of enhancing their quality of life, and increasing integration into their community Key Responsibilities for the
Support Coordinator:
Planning Supports Establish a trusting relationship with the individual being served and their team Identify the needs of the person being supported Collaborate with the individual's team to develop an Individualized Service Plan that will support the person in meeting identified needs Linking Resources Research community resources and funding programs Link individuals to needed resources, through providing information, making referrals, and coordinating meetings Quality Enhancement Meet with individuals and their planning team, following state and federal frequency requirements Monitor and assess the quality, timeliness, and effectiveness of services and supports throughout the individual's plan year Documentation Maintain privacy and confidentiality of all protected health information/electronic protected health information, in accordance with state and federal guidelines Complete Medicaid billing documentation for case management services, with an average target of 120 hours of billing each month Complete required forms and documentation, in accordance with Department of Mental Health guidelines, Medicaid regulations, and LCBDD policies

Support Coordinators will receive further training on job tasks during orientation, as well as updates to their training throughout their employment. Additional job tasks may be assigned as needed.
