Job Description

Overview:
We are seeking a compassionate and experienced Social Services Assistant to join our team at Kalispell Rehab. As the Social Services Assistant, you will play a crucial role in enhancing the quality of life for our residents and their families. You will assist our social work team in providing comprehensive services to support the physical, emotional, and social well-being of our residents.
Responsibilities:
Oversee the daily operations of the social work department, ensuring the delivery of high-quality care and services. Develop, implement, and evaluate social service programs to see the needs of residents. Conduct psychosocial assessments and develop individualized care plans in collaboration with interdisciplinary teams. Provide counseling and support to residents and families, addressing emotional and adjustment issues related to long-term care. Advocate for residents' rights and ensure ethical and culturally sensitive practices. Coordinate discharge planning and facilitate transitions to other levels of care or community resources. Maintain accurate and confidential resident records, complying with regulatory standards and guidelines. Collaborate with community organizations, agencies, and healthcare professionals to enhance resident care and resources.
Qualifications:
Minimum 1 year of experience in social work, preferably in Long-term care.
