Job Description

Veteran Support Coordinator (Case Manager) The Poverello Center, Inc. The Poverello Center is seeking fun, creative, and empowering individuals to support our mission of offering food, shelter, help and hope to all who ask in the Missoula community. Specifically, we are seeking an independent worker who is interested in serving on a diverse team, engaging in creative problem solving, and empowering those around them. This dynamic opportunity provides direct services and case management to residents within our Veterans' transitional housing and shelter programs. An award winning non-profit organization, the Poverello Center provides education, advocacy, and a multitude of services to improve the health, well-being, and stability of people struggling with poverty in Missoula. We are dedicated to treating people with dignity and respect, regardless of their housing status and envision a collaborative and supportive community in which everyone has a safe place to call home. We are committed to promoting and providing a range of services to improve the health and security of people experiencing houselessness in the Missoula community. Why We Need You The ideal candidate has a passion and talent for helping others, specifically veterans, communicating clearly and compassionately, and has the ability to connect and empathize with individuals who are experiencing the effects of poverty, mental health, and/or substance use disorders. They remain calm in high stress situations and encounters. They can manage completing and processing paperwork in a timely manner. Lastly, they are trustworthy, dependable, relatable and friendly. What You'll Do The Veteran Case Manager reports to the Veteran Programs Manager and is responsible for the following:
  • Establishes professional rapport and maintains healthy, supportive boundaries with residents.
  • Provides direct services and case management to Housing Montana Heroes and Veteran Shelter Program residents using treatment plan format consistent with the Poverello mission and veteran program policies.
  • Helps residents to develop skills and self-sufficiency, works with residents to obtain permanent housing, executes social activities and outings, and supports veterans in obtaining and maintaining critical services pertinent to their overall well-being; including physical and mental health care and substance abuse treatment, as needed by the resident.
  • Writes and maintains timely, and objective case notes for all residents on caseload.
  • Maintains pertinent communication and keeps accurate logs of all contact and actions relating to candidate screening both internally and via the Homeless Management Information System (HMIS).
  • Communicates resident issues and notices to other Veteran and Shelter staff and the VA Grant Per Diem Liaison.
  • Supportively and therapeutically confronts residents regarding program violations, writing incident reports due to inconsistent completion of responsibilities, and behavior plans.
  • Participates in the Missoula Coordinated Entry System (MCES) by attending Case Conferencing and other meetings, screening candidates referred from this system, and staying up to date on best practices within MCES.

Experience:
Bachelor in Social Work (BSW) or equivalent education and experience preferred. Experience working with veterans and people who are houseless or in poverty is preferred. This position is full time and is based on 40 hours per week with at least three shifts being 11am-7pm (exact schedule to be determined upon hire) and may require occasional weekend availability. This position reports to the Director of Programs and is based at the emergency shelter in Missoula, Montana. The wage for this position is $20-22 hourly DOE. In addition, this position includes the following benefits: 1.
