With a growing presence in New Jersey, our team is driven by compassion, professionalism, and a commitment to making a measurable impact in the lives of others. Role Summary We are seeking a full-time Support Coordinator to join our team. In this role, you will manage a caseload of individuals with developmental disabilities, helping them access and maintain services in their communities. You'll serve as a vital advocate and liaison between individuals, families, providers, and state systems. Key Responsibilities Coordinate and monitor services for individuals with developmental disabilities Conduct intake meetings and develop person-centered service plans Facilitate the initiation of services with approved state vendors Maintain monthly contact and ensure services align with individual needs Provide emergency on-call support as needed Accurately document and update information in the Consumer Information System (CIS) Required Qualifications Bachelor's degree in Social Work, Psychology, Sociology, Counseling, Gerontology, Special Education, or Rehabilitation Minimum 1 year of experience working with individuals with intellectual/developmental disabilities Proficient in Microsoft Word and general computer applications Valid driver's license and proof of auto insurance Preferred Qualifications BOGGS Support Coordination Orientation Certificate Why Join Us?
