Job Description

COME JOIN OUR TEAM!
Summary We are seeking a compassionate and dedicated regular, full-time Community Support Worker to join our team at La Casa Family Health Center. The Provider shall administer the Linkages Supportive Housing Program to eligible consumers within Chaves County, after being referred to as the Provider's geographic service area.' Eligible consumers shall meet the criteria as outlined in the Linkages Program Manual. The Provider shall have the necessary administrative capacity for the local implementation, administration, and delivery of the program in accordance with the Scope of Work, and all other guidelines and procedures set in the Linkages Program Manual.
Job Duties:
1. At minimum, the required training for a CSW shall include: 20 hours of initial documented training from the list provided by Health Care Authority within 30 days of employment. 2. The CSW is responsible for maintaining consumer records which demonstrate consistent compliance with documentation requirements of all interventions, location of services, time of services and the CSW's name, credentials and date of service delivery. 3. CSW's as assigned, will be responsible for assisting any adult in the Linkages housing program designated to La Casa Community Behavioral Health Services. Activities within those roles will be defined by the most current Scope of Work Provided. 4. Receive, screen and determine eligibility of Linkages applicants. 5. Support clients with hotel/motel vouchers upon funding availability during the interim period of voucher issuance to filled status, per Linkages Program Manual. 6. Support clients in need with furniture/furnishing stipends, per Linkages Program Manual. 7. Assist clients with successfully maintaining their tenancy and progress toward self-sufficiency 8. Render CCSS, as necessary and applicable to clients' needs and submit separate billing to be reimburses via CCSS and/or other Medicaid reimbursable services. 9. Conduct community outreach in the geographic service area. 10. Collaborate with the Housing Administrator to ensure that all rental units funded through the Linkages Program have a signed legal rental agreement/lease and an approved Housing Quality Standards (HQS) inspection or National Standards for the Physical Inspection of Real Estate (NSPIRE) prior to funding of the rental assistance voucher for the housing unit and the consumer moving into the housing unit. (BHSD will notify Linkages Housing Administrator providers when NSPIRE will be effective for Linkages programming) 11. In addition, could potentially carry a CCSS Adult caseload (under 10)
Other Duties:
1. Ability to physically restrain violent/abusive individuals in a safe and secure manner. 2. Ability to lift 25 pounds.
Qualifications:
1. At least 18 years of age and 2. Bachelor's degree in a human service field from an accredited university and one year of relevant experience with the target population or 3. Associate's degree in a human service field from an accredited college or university and a minimum of two years of experience working with the target population or 4. High school graduation or GED and a minimum of three years' experience working with the target population. 5. Possesses a valid New Mexico Driver's License.
Working Conditions:
The CSW will perform a minimum of 60% of case work in vivo , or in the environment that best reflects the day-to-day life of the consumer. The CSW must be able to sit and stand for the duration of the treatment period (up to two hours) and must be sufficiently mobile to reach the consumer's setting. The CSW must be able to use standard office equipment (telephone, fax machine, copy machine, computer, etc.). The CSW must also demonstrate excellent verbal and written communication skills.
