Job Description

The Resident Advocate supports the Social Services department by promoting resident rights, enhancing resident quality of life, and assisting residents and families with communication, resources, and care coordination throughout their stay. This position serves as a supportive liaison between residents, families, interdisciplinary teams, and community resources to help ensure resident concerns are addressed promptly, respectfully, and in accordance with facility policies and regulatory requirements. The Resident Advocate is not responsible for clinical decision-making but works collaboratively with Social Services and the interdisciplinary team to support resident satisfaction, engagement, and overall well-being. Essential Duties & Responsibilities Build positive relationships with residents and families through regular rounds and ongoing communication. Support Social Services with resident admissions, orientation, transitions, and discharge planning activities as assigned. Assist residents and families in understanding facility services, resident rights, grievance processes, and available resources. Receive, document, and appropriately escalate resident or family concerns to Social Services leadership and/or department heads. Follow up on concerns to promote timely resolution and resident satisfaction. Assist with the completion, tracking, and organization of social services documentation and records. Support coordination of care conferences and assist with communication to families regarding scheduling and participation. Collaborate with nursing, activities, dietary, therapy, business office, and leadership teams to address non-clinical resident needs. Help connect residents and families to community resources, benefit programs, and supportive services as appropriate. Conduct resident check-ins and satisfaction rounds and communicate trends to Social Services leadership. Support resident adjustment to the facility environment and encourage engagement in activities and services. Maintain confidentiality and comply with HIPAA and resident privacy requirements. Participate in surveys, audits, quality improvement initiatives, and departmental meetings as requested. Assist with special projects and other duties as assigned.
Pay:
$20.00 - $24.00 per hour
