Most usually scheduled for Four 6-hour daytime shifts, could include weekends and sometimes includes attending events in-person.
SUMMARY:
The Outreach Worker will support the Program Manager with the coordination of community-based support and engagement services to individuals and families throughout Wayne County. The Outreach Worker will manage established partnerships to ensure schedules; referrals and marketing protocol are supported. The Outreach Worker ensures events are visible on all platforms, including but not limited to, websites, social media and print with distribution to local agencies, health centers, retailers and town chambers.
RESPONSIBILITIES
Conduct outreach activities to identify and engage individuals in need of services. Develop and maintain relationships with local organizations, agencies, and community leaders. Organize and participate in community events, workshops, and educational sessions. Maintain accurate records and submit required reports in compliance with grant requirements. Travel throughout rural areas to attend community meetings. Provide information and referrals to community resources, health programs, and social services.
REQUIRED QUALIFICATIONS
Associate's degree
PREFERRED QUALIFICATIONS
3 years of administrative office experience in healthcare. Excellent oral and written communication and interpersonal skills. Knowledge of rural community resources and challenges. Strong organizational and problem-solving skills. High level of proficiency with PC, software applications, and reporting tools.
