This position offers a flexible schedule of approximately 3 to 4 days per week, making it ideal for someone seeking work life balance while making a meaningful impact on the lives of residents and families. Responsibilities Assist with resident admissions, care conferences, scheduling and discharge planning Meet with residents and families to address concerns, provide support, and connect them with community resources Complete psychosocial assessments and required documentation Participate in and schedule interdisciplinary care planning meetings Support resident rights and assist with grievance resolution Coordinate discharge and ancillary services as needed Collaborate with nursing, therapy, activities, and administration to meet resident needs Assist residents and families with advanced directives and long-term planning discussions Qualifications High school diploma or equivalent required Experience in healthcare, long term care, social services, admissions, activities, case management, customer service, or a related field preferred Strong communication, organization, and problem-solving skills Ability to work independently and manage multiple priorities Basic computer skills such as Microsoft office programs required Schedule Flexible schedule Approximately 3 to 4 days per week with occasional weekend hours Day shift hours with flexibility based on resident and facility needs
