This role provides support with care planning, resident advocacy, discharge planning, and family communication.
Qualifications:
Prior experience with social services is required Previous experience in Skilled nursing, long-term care, or similar setting preferred Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred (or equivalent experience) Strong communication, interpersonal, and organizational skills Compassionate, patient-centered approach Knowledge of long-term care regulations is a plus Ability to maintain confidentiality and professionalism
Key Responsibilities:
Assist with admissions, transfers, and discharge planning processes Support residents and families with adjustment to facility placement Help coordinate appointments, transportation, and community resources Maintain accurate social service documentation in compliance with state and federal regulations Participate in care plan meetings and interdisciplinary team collaboration Address resident concerns, grievances, and psychosocial needs Assist with Medicaid applications, insurance paperwork, and resource referrals as needed Provide emotional support and advocacy for residents and families Ensure resident rights are upheld at all times Apply Today!
