Valid U.S. driver's license isREQUIRED
Must have reliable access to a personal vehicle Applicants WITHOUT a driver's license and vehicle will NOT be considered Must be 21+ and pass a driving background check Auto insurance required if using a personal vehicle Vinfen is seeking a Clinical Care Outreach Worker to work collaboratively with the Clinical Care Manager (CCM) to support individuals enrolled in One Care with complex healthcare needs. This role provides care coordination, outreach, and administrative support in both community and office-based settings. Key Responsibilities Conduct outreach and engagement with One Care participants Support care coordination and implementation of Individual Care Plans Complete and maintain accurate documentation and care plans Collaborate with healthcare providers, community resources, and Vinfen teams Accompany and transport participants to medical appointments as needed Support health education and wellness self-management Participate in quality improvement and team meetings Requirements High school diploma or equivalent required (Bachelor's degree preferred) Minimum of 2 years of human services experience required Experience with psychiatric disabilities or care coordination preferred Strong organizational, communication, and documentation skills Ability to work independently and as part of a multidisciplinary team Apply today to join a mission-driven team improving access to care and health outcomes in the community.Pay:
