Intellectual Developmental Disability Support Coordinator

Job Description Neighbours, Inc., is an innovative non-profit organization created specifically to support people with disabilities and their families in choosing and designing a life for themselves. We are expanding our team of creative, team-oriented people to facilitate person-centered planning and support Self-direction with individuals and families in the framework of the Division of Developmental Disabilities Systems Change. Responsibilities and Duties Directly meeting with people Complete Quarterly & Monthly Monitoring Tools Assist individuals and their families in identifying support needs, current available resources, and their vision for the future. Maintain accurate, complete and up-to-date contact notes, data, and other required records and complete and submit required reports by established deadlines. Flexible working hours Must have strong computer skills, proficient with Microsoft Word, Excel, and PowerPoint. Valid Driver's license and the ability to travel the State of New Jersey Qualifications and Skills Bachelor's degree and 1-year full-time equivalent experience working in any capacity with adults (age 21 or older) with Intellectual Disabilities required.
