$43,888$ About Us The Columbus Organization supports individuals with complex care needs through person-centered services that promote independence, community inclusion, and improved quality of life. Position Summary The Support Coordinator assists individuals with intellectual and developmental disabilities in accessing, coordinating, and monitoring community-based services. This role focuses on care coordination, service planning, and ongoing support to help individuals achieve personal goals and maintain quality outcomes. Key Responsibilities Coordinate and manage individualized service plans (ISP) Conduct assessments and gather clinical/social information Facilitate ISP meetings and ensure annual updates Connect individuals to community resources and services Assist with Medicaid eligibility and recertification Maintain monthly face-to-face contact per guidelines Monitor service delivery and outcomes Document services and maintain compliance with DBHDD and HIPAA Respond to participant needs, concerns, and crises Collaborate with families, providers, and interdisciplinary teams Qualifications Bachelor's degree in human services field (required) 1+ year experience in case management or developmental disabilities services Master's degree may substitute for experience Valid driver's license and ability to travel locally Must reside within assigned coverage area (preferred) Skills Care coordination and case management Strong communication and relationship-building Problem-solving and critical thinking Knowledge of Medicaid/waiver programs Microsoft Office and documentation skills Work Environment This is a mobile, field-based position requiring travel throughout the assigned counties. Work includes home visits, community settings, and occasional remote/administrative tasks. Flexibility for evenings or weekends may be required.
