Non-Exempt Encompass Community Supports, formerly known as Rappahannock Rapidan Community Services (RRCS) is hiring a Support Coordinator II- Developmental Disabilities.
Encompass Community Supports is a Virginia State community services organization that improves the quality of life in Culpeper, Fauquier, Madison, Orange, and Rappahannock Counties by providing comprehensive mental health, substance use, developmental disability, housing, children's, and aging services. Our Support Coordinator
- Developmental Disabilities assesses clients with developmental disabilities by assessing needs, linking to resources and services, coordinating among service providers, and monitoring the effectiveness of services.
Work is performed at various community locations as well as home visits, including locations outside of catchment, but takes place during the workday. Documentation completion may include some office time and telework options. Work is performed under regular supervision.
SOME OF THE ESSENTIAL FUNCTIONS
Facilitates, assesses and evaluates client services and needs in the home, hospital or clinic setting; presents options and services based upon a needs assessment. Develops an individual service plan of care and services appropriate to the evaluation with the client/family consensus; reviews and updates service plans. Refers, links, and coordinates to multiple agencies to ensure appropriate service initiation and delivery. Ensures programs operational compliance to all regulatory guidelines. Prepares and maintains client charts and other records and documents all activities performed on clients' behalf. Prepares applications and variety of reports related to benefits and potential funding sources to meet client needs. Advocates for clients rights, needs and access to services while ensuring human rights and satisfaction with all services. Assists with discharge planning from care facilities and educational programs. Provides linkage to crisis intervention and prevention services. Attends meetings, staffings, and conferences as they relate to client, staff and program needs. Attends inter-disciplinary team meetings to discuss decisions for client plan of care. KNOWLEDGE, SKILLS AND ABILITIES
General knowledge of community and agency resources and programs of regional and state sources for the community service population. General knowledge of interviewing and supportive counseling techniques. General knowledge of social casework techniques; general knowledge of human development and behavior. General knowledge of the theories, principles and techniques of individual, family and group therapy. Ability to analyze facts and exercise sound judgment in arriving at conclusions. Ability to communicate complex ideas effectively, orally and in writing. Ability to prepare clear and concise reports. Ability to establish and maintain effective working relationships with clients, associates and the general public. Knowledge of MS Office, to include Word and Excel.
EDUCATION AND EXPERIENCE
Bachelor's Degree in Human Services or equivalent, At least 1 year working with individuals with developmental disabilities is required. Fluent in Spanish is Preferred.
PHYSICAL REQUIREMENTS
This work requires little or no physical effort or exertion.
SPECIAL REQUIREMENTS
Valid Virginia driver's license; Attends training and maintains certifications as required by the Agency and regulatory guidelines, such as CPR, First Aid, Person-Centered Plan, and DBHDS Case Management Modules.
