Minimum Data Set (MDS) - Social Services Assistant Long Term Care

Elk River Health & Nursing Center of Ardmore seeks Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) to serve as Social Services & Minimum Data Set (MDS) Assistantant. The successful candidate will provide compassionate support. Registered Nurse (RN) - Licensed Practical Nurse (LPN) Monday through Friday 8:00 AM - 4:30 PM Equal Employment Opportunity Responsibilities

• Assisting with PASRR's, Consents, and UDAS
• Providing family support and guidance
• Gathering supporting documents for MDS completion
• Assisting with care plan updates and care conference preparation
• Assisting with Section GG documentation review and supporting documentation validation
• Assisting with discharge assessments
• Assisting with resident interviews (BIMS, PHQ, and pain interviews)
• Updating care plans accordingly
• Reviewing progress notes, identifying diagnoses, and entering new notes into PCC
• Weekly updating of 802 forms This position is ideal for motivated individuals.

Pay:

$27.00 - $40.00 per hour
