Identify social and emotional needs for residents. Provide medically related social services to attain or maintain the highest practical physical, mental, and psycho-social well-being of each resident.

Responsibilities:

Assists in completing resident admission tasks including correspondence and facility tours. Schedules the transfer of residents to/from home or to another facility. Serves as the Ombudsman for the residents. Assists the Director of Social Services in resident council meetings, care plan meetings, and other appropriate committee meetings. Completes social service assessments for each resident resulting in Individualized Care Plan. Records accurate and timely social service progress notes Schedules outpatient appointments, including transportation. Ensures residents' personal needs are met

Qualifications:

Degree in social work preferred Two years of experience working in geriatrics preferred State Licensure preferred Previous experience with Nursing Homes preferred.
