Job Description

Job Summary:
Seattle Medical Post Acute Care in Seattle , Washington is seeking a Social Services Assistant to provide direct support the Social Services Director in identifying and providing for each resident's social, emotional, and psychological needs, and the continuing development of the resident's full potential during their stay at the facility and when appropriate, transition back to the community. Essential Functions include, but not limited to: Assist with Admission, Quarterly and Discharge Care Conferences with the resident and family/responsible party. Conduct and document a Social Services Admission, Quarterly and Comprehensive Evaluations and interviews. Assist with discharge planning. Arrange ancillary services, as ordered, needed, or requested. Provide/arrange for Social Service, as indicated by Resident/Responsible Party needs, concerns, or changes. Review complaints and grievances made by the resident and follows facility's established policy and procedure. Works under the direction of the Social Services Director. Act in compliance with all corporate policy, state, federal, and other regulatory standards.
Education, Licensure, & Experience Requirements:
Must have a minimum of an Associate's Degree and meet State certification requirements. A minimum of 1 to 2 years working in geriatrics.
Preferred :
Experience in Behavior Add-on Programs.
Knowledge, Skills & Abilities:
Excellent written and oral communication skills. Must be able to sit or stand in meetings 1-3 hours at a time. Point Click Care knowledge is a plus.
