Social Services Assistant Aurora Valley Care - 3.4 Spokane Valley, WA Job Details Full-time $23 - $25 an hour 1 day ago Qualifications High school diploma or GED Full Job Description At Aurora Valley Care Skilled Nursing & Rehab (a Caldera Care facility), we are dedicated to creating a warm and welcoming environment that feels like home for both our residents and our staff. We are currently seeking a dedicated and compassionate Social Services Assistant (SSA) possessing high leadership skills and who is looking to be challenged in their career. As the SSA you will be joining a family of highly skilled and motivated individuals within the skilled nursing setting who aim to provide the best quality of care to our residents and a caring working environment to our staff. Responsibilities:
Assess and implement social services care plans for all residents Work closely with other departments to coordinate social services support for residents Coordinate quarterly care conferences with residents/responsible parties, and as needed Ensure the timely completion of social services evaluations, progress notes, and MDS tasks Facilitate resident discharges and coordinate all discharge needs (i.e. Home Health, DME, DSHS, etc.) Communicate with community vendors and DSHS Case Managers on a daily basis Provide behavioral and emotional support to residents as needed Attend meetings as required Additional Duties Participate in training programs Assist Social Services Director as needed Interdisciplinary tasks as assigned
Requirements:
Minimum 1 year of experience in a long term care setting preferred, though not required. High School Diploma required
