Job Summary The Peer Recovery Coach provides non-clinical services to support patients in establishing and maintaining recovery from high-risk alcohol and drug use. As a valued member of the hospital team, the Peer Recovery Coach offers peer support, motivation, and guidance to encourage positive behavioral change. This role includes direct patient interventions in the Emergency Department and other hospital units, as well as community outreach and engagement following hospital discharge. The Peer Recovery Coach develops individualized service plans to promote successful linkage to treatment and recovery support services.

Schedule:

Part-time, 20 hours per week (days and times to be determined) Key Responsibilities Patient Support & Intervention Conduct brief interventions for patients who screen positive for risky alcohol and/or drug use. Assess patient history, treatment background, motivation for change, and community support needs. Utilize motivational interviewing techniques to support recovery planning. Assist patients in setting personal recovery goals. Act as peer support throughout hospitalization and post-discharge. Provide recovery education and materials to patients and families. Notify the patient's nurse prior to patient visits. Care Coordination & Community Outreach Develop service plans aligned with patient needs and recovery goals. Coordinate referrals and connect patients with community treatment and support services. Reduce barriers to care by assisting with appointment scheduling, transportation, and follow-up. Conduct telephone and face-to-face outreach to ensure continued engagement. Verify attendance at treatment appointments and follow up on missed visits. Conduct community and street outreach to locate and re-engage patients when needed. Collaborate with hospital staff, social work, and community agencies. Team Collaboration & Professional Standards Promote hospital and department policies and procedures. Maintain patient privacy and confidentiality. Serve as a recovery support resource for clinical team members. Participate in required training, meetings, and documentation. Demonstrate positive, professional communication with patients, families, and colleagues. Minimum Qualifications

Education:

High school diploma or GED required.

Experience:

Must be actively engaged in a personal recovery program. Minimum of two (2) years of demonstrated personal recovery and sustained abstinence from alcohol and/or drug use.

Licensure/Certification:
