La Palma Nursing Center is a 72 bed Skilled Nursing Facility in Anaheim with a reputation for top notch care. We are looking for a Director of Social Services who can help lead the team and serve our diverse resident population. Korean and English speaking will be a strong plus as there is a significant Korean speaking population in the facility. Responsibilities Oversee, plan and implement the social services programs of this facility. Organize and participate in interdisciplinary care plan meetings to ensure treatment of social and emotional needs. Participate in discharge planning, development and implementation of social care plans and resident assessments. Maintain accurate records and documentation in compliance with facility and regulatory standards.
