The Social Services Director will work closely with Residents, families, and community resources to provide social services support to individuals and families in need.

Responsibilities:

Provide case management services to individuals and families in need of social services support Conduct assessments of client needs and develop individualized care plans Coordinate and connect patients with community resources and services, including housing, healthcare, employment, and financial assistance Conducting needs assessments to ascertain physical, cognitive, and emotional abilities. Advocate for clients and ensure that their rights are protected Maintain accurate and up-to-date client records and case notes Collaborate with other social services agencies and community organizations to provide comprehensive services to clients Participate in community outreach and education efforts to raise awareness of available social services resources

Requirements:

Bachelor's degree in social work, psychology, or a related field (preferred) Minimum of 2 years of experience in social services or related field (preferred) Strong communication and interpersonal skills Ability to work independently and as part of a team Proficient in Microsoft Office and database management Ability to maintain confidentiality and professionalism at all times This is a full-time position with a competitive salary and benefits package.
