Social Services Director will support residents' emotional and psychosocial needs while they are in our facility. This role focuses on making sure residents feel comfortable, respected, and understood. The director works closely with nurses, MDS, therapists, and families to make sure each resident's personal needs and preferences are included in their care. Social Services Director manages many responsibilities, including discharges, and resident and family concerns. Duties Conduct comprehensive psychosocial assessments upon admission and at required intervals Develop, implement, and update individualized care plans with the interdisciplinary team Complete and document MDS sections related to mood (PHQ-9) and resident preferences Ensure all documentation meets regulatory and compliance standards Coordinate discharge planning, including safe transitions to home or other care settings Arrange services such as home health, hospice, and durable medical equipment Provide education and support to residents and families during discharge process Offer emotional support and counseling for adjustment, mental health, and end-of-life concerns Facilitate and participate in care conferences with residents, families, and staff Advocate for resident rights and ensure dignity and person-centered care Manage, document, and resolve resident grievances and complaints Participate in abuse/neglect investigations and reporting as required Communicate regularly with families regarding resident status and care needs Collaborate with community resources and external service providers Support admissions process by gathering social history and assessing needs Maintain survey readiness and ensure compliance with all applicable regulations
