The Director of Social Services will play a vital role in ensuring that residents receive emotional support, counseling, and assistance with care planning, while also working closely with families and the interdisciplinary team to provide the highest quality of care. Responsibilities Plan, develop, and implement social service programs to meet the emotional and social needs of residents. Serve as a liaison between residents, families, and facility staff to ensure effective communication and care coordination. Assist residents and families in understanding and adjusting to illness, disability, or placement in a long-term care facility. Provide support with care planning, discharge planning, and transitions of care. Maintain compliance with state, federal, and facility regulations. Document social service assessments, progress notes, and care plans accurately and timely. Supervise and mentor social services staff, promoting a positive and supportive team environment. Requirements Bachelor's degree in Social Work (BSW) required; Master's degree preferred. Current state license (LSW or equivalent) in good standing. at least 35-38 hours of experience Knowledge of federal and state regulations related to long-term care. Strong communication, counseling, and leadership skills. Ability to work collaboratively with residents, families, and interdisciplinary teams.
