Role:
The Social Services Director is responsible for planning, developing, organizing, implementing, evaluating, and directing the social service programs of the facility in accordance with current federal, state, and local standards, guidelines, and facility policies and procedures. This role ensures that the medically related emotional and social needs of each resident are identified, addressed, and maintained on an individual basis. The Social Services Director works closely with residents, families, interdisciplinary teams, and community resources to promote resident well-being, advocate for resident rights, and support quality care within the skilled nursing facility.Qualifications:
Bachelor's degree in Social Work required; or Bachelor's degree in a human-services related field including sociology, gerontology, psychology, rehabilitation counseling, or special education Minimum of one (1) year of supervised social work experience in a healthcare setting Skilled nursing facility experience preferred Strong communication, organizational, and interpersonal skills Knowledge of resident rights, discharge planning, and regulatory compliance preferredResponsibilities:
Plan, develop, organize, implement, evaluate, and direct the facility's social service programs in accordance with applicable federal, state, and facility regulations Participate in discharge planning, care plan development, and resident assessments Conduct resident and family interviews as needed to assess needs and concerns Maintain Social Service progress notes documenting response to care plans and adjustment to facility life Assist residents and families in accessing community social, health, and welfare resources Maintain communication with families regarding resident progress (non-medical updates) Assist in resolving grievances from residents, families, responsible parties, or ombudsmen Collaborate with the interdisciplinary team to support resident care and servicesBenefits:

The Social Services Director must possess strong communication and interpersonal skills to effectively support residents, families, and interdisciplinary teams. Experience in case management, discharge planning, psychosocial assessments, and care planning is essential to ensure residents receive appropriate services and support. The role requires strong problem-solving, crisis intervention, and conflict resolution abilities, along with a compassionate and resident-centered approach to care.
