Full Job Description Social Services Director Position Summary:
Manages the Social Services Department in accordance with all regulations for the industry. Evaluates, plans, develops, organizes, implements, directs and ensures the medically related emotional and social needs of patients/residents are met and maintained on an individual basis.
Qualifications:
Bachelor's degree or equivalent education/experience Positive, encouraging personality who can professionally manage difficult emotions/persons Experienced in the development and implementation of social care plans, patient assessments and discharge planning Outstanding communication (verbal and written) skills required Minimum of 2 years' experience in hospital or skilled nursing setting required Ability to multi-task, prioritize work, create organized files and maintain accurate records Proven success in supporting patients/residents, families and staff in a kind, positive and professional manner, adapting your communication style to their needs
Responsibilities:
Develops a comprehensive social history and assessment of residents that includes problems and strengths, special needs and preferences (social, religious and/or cultural) and how to best address and support them for the care plan Assists patients/residents and their caregivers with transitioning to new environments, orienting them to the facility and our services, educating on resident rights and other aspects of transitioning to/from care facility Acts as mediator to resolve concerns/conflicts that may arise regarding care and treatment, admission and discharge procedures, physician care and/or any other situations that could impact or negatively affect the patient/resident Participates in the development of individual's care plans including the goals and approaches for Social Services Actively participates in the coordination of care services, evaluating the ongoing needs of residents (attends resident care plan meetings, rehabilitation and resident/family meetings, departmental staff meetings, annual policy review meetings, monthly quality assurance meetings, etc.) Ensures Advanced Directives are completed, maintained and placed in resident charts when needed Develops a comprehensive discharge plan in coordination with residents/families and multi-disciplinary staff Coordinates discharges/transfers to ensure appropriateness and continuity of care Establishes and maintains positive relationships with residents, family members, physicians, community leaders, state and federal surveyors, hospital case managers and other affiliated professionals Identifies and provides referral, agency and/or community resources to meet patient/family needs Coordinates Support Groups, participates in policy decisions (as they affect resident life issues) and advocates for Residents Rights Completes all mandatory documentation timely and accurately as established by applicable regulations Maintains confidentiality of all admission information and resident records as required Leads by example, remaining calm and professional in all work-related situations
