Social Services Director (SSD)

We are looking for a Social Services Director (SSD) who can work in a fast-paced environment, have a positive attitude, and work together as part of a team. •Starting at $36-$42/hr (pending experience), FULL TIME-Monday-Friday, 830am-5pm•(Shift days/time can be flexible).

Responsibilities:

Develop safe discharge plans with DME, follow up appointments, etc. Assist residents in achieving the highest practicable level of self-care, independence and well-being. Provide support so that the highest physical, mental and psychosocial wellbeing of each resident is attained or maintained, update care plans, and discuss barriers to goals. Complete assessments, host care conference with IDT and use electronic charting to document on residents. Manage Social Services Team, be a team player.

Requirements:

Must Experience in Social Services in skilled nursing facility and familiarity with electronic charting
