Case Manager (Human Services) Atlantic Group - 3.6 San Diego, CA Job Details Full-time | Contract $22 - $26 an hour 1 day ago Qualifications Teamwork Case management Motivational interviewing Section 8 Child welfare Full Job Description Job Summary In this role, you will coordinate comprehensive support services, develop personalized care plans, and connect clients with essential resources to foster stability and growth. Your proactive approach and empathetic communication will empower clients to overcome obstacles, improve their well-being, and achieve their goals. Duties Conduct thorough assessments to identify clients' needs across behavioral health, child welfare, housing, and workforce development sectors Develop individualized care plans that incorporate crisis intervention, addiction counseling, and motivational interviewing techniques Coordinate services such as Section 8 housing assistance, Medicare support, and childcare programs to ensure seamless access for clients Provide crisis management and intervention during urgent situations, offering immediate support and stabilization strategies Mentor clients through life challenges by offering guidance, behavioral health education, and social work expertise Supervise and collaborate with team members to ensure consistent delivery of quality care and client advocacy Maintain detailed documentation of client interactions, service plans, progress notes, and relevant reports in compliance with organizational standards Experience Proven experience in social work or case management roles within behavioral health or child welfare settings Strong background in crisis intervention, crisis management, and addiction counseling techniques Demonstrated ability to handle sensitive situations with professionalism and empathy while maintaining clear boundaries Supervising experience is preferred to effectively lead team efforts and support junior staff members Familiarity with programs such as Section 8 housing, Medicare, workforce development initiatives, and child welfare systems Excellent communication skills combined with motivational interviewing expertise to engage clients effectively Knowledge of community resources related to childcare services, mental health support, and social services programs #
