Within the Case Management service line, the Nurse Manager of Case Management will guide an integrated approach to care coordination, utilization management, and discharge planning that ensures seamless transitions, appropriate levels of care, and reduced avoidable days; operating in a high-performing, accredited facility currently undergoing a multi million dollars expansion, focused on providing comprehensive, local access to emergency, trauma, and advanced care, the Case Management team leverages optimized EHR workflows, interdisciplinary rounds, quality dashboards, and strong payer collaboration to advance throughput, reduce readmissions, and champion patient advocacy at every step.

Reporting into senior nursing and population health leadership, the Nurse Director of Utilization Review and Discharge Planning will lead a supported team of case managers, social workers, utilization review nurses, and care coordinators while partnering closely with hospitalists, surgeons, ED leaders, and ancillary departments; responsibilities span strategic planning, regulatory readiness, denials prevention, length-of-stay optimization, care transition innovation, budget stewardship, talent development, and community resource integration—fueled by friendly nurses and a diverse healthcare professional team that values mutual respect, shared goals, and frontline-driven process improvement.
