Social Service Director

Social Service Director White Blossom Care Center Full-Time | $35/hour + Benefits White Blossom Care Center is seeking a compassionate, experienced Social Service Director to join our team. In this leadership role, you will oversee and coordinate social services for our residents, ensuring their psychosocial needs are met with dignity, respect, and high-quality care. Key Responsibilities

• Develop and implement comprehensive social service programs for residents
• Conduct assessments, care planning, and ongoing evaluations of resident needs
• Provide counseling and emotional support to residents and families
• Coordinate discharge planning and community resources
• Advocate for residents' rights and ensure regulatory compliance
• Collaborate with interdisciplinary team members to deliver holistic care
• Maintain accurate documentation in accordance with state and federal guidelines Qualifications
• Bachelor's degree in Social Work or related field (Master's preferred)
• Valid Social Services Designee certification (as required by state)
• Previous experience in a skilled nursing or long-term care setting preferred
• Strong communication, leadership, and organizational skills
• Knowledge of federal and state regulations for long-term care Compensation & Benefits
