We serve all of San Joaquin and Stanislaus Counties. We are seeking a Dynamic, self-motivated Senior Manager of Social Services (SMSS) who thrives in a fast-paced environment and supports our mission . This is a Full-Time position reporting to the Chief Clinical Officer (CCO).
POSITION PURPOSE/SUMMARY
Responsible for implementing the psychosocial components of Hospice of San Joaquin's program of care and supervising staff assigned to provide social services, including social workers, chaplains, and the bereavement department.
ESSENTIAL DUTIES AND RESPONSIBILITIES/ACCOUNTABILITY
1. Responsible for orienting, supervising, and evaluate staff assigned to social service programs. 2. Anticipates staffing needs and modifies schedules appropriately. Maintains productivity and consistency with hospice practice. 3. Provides ongoing direction for social service staff regarding changes in clinical policies and procedures. 4. Meets with social services staff monthly to review projects, plans, problems, and program activities. 5. Applies the established principles of social work in directing staff in managing clinical plans, interventions, and outcomes. 6. Coordinates Saturday and Holiday rotations of social service staff. 7. Performs chart audits of staff documentation and provides education as needed. 10. Provide LCSW supervision for ACSW seeking LCSW licensing. 11. Serves as a leadership resource for crises and urgent operational issues, including availability outside of normal business hours, as needed, to provide consultation, support, and direction to staff. 12. Develops, conducts, or supervises social service quality improvement activities and implements needed corrections. 13 Works with the CCO to develop an annual budget for programs and supplies. 14. Reports periodically to the Chief Officers regarding the performance of social services. 15. Completes an annual report on department activities. 16. Collaborates care with social service staff departments, including bereavement, home care, palliative, the hospice house, and the volunteer department. 17. Ensures employees have access to 1:1 support and monthly grief group access. 18. Educates IDT members regarding psychological, social, and spiritual issues related to death and dying. 19. Acts as a community advocate for Hospice through ongoing speaking engagements regarding Hospice services. 20. Attends IDT meetings and actively participates in discussions related to patient/family care, including the psychosocial, spiritual, and support areas of need. 21. Recognizes the knowledge and skills of other disciplines represented on the team and advocates for the most appropriate team member(s) intervention(s) with the patient/family. 22. Reviews team summaries to ascertain that all interventions and timelines for social service intervention are managed and completed. 23. Ensures social service staff provide emotional support at IDT and as needed to our employees 24. Identifies personal education needs, seeks formal programs, participates in professional educational activities, and seeks other avenues (i.e., reading articles/books, etc.) to meet these needs. 25. Demonstrates and develops a leadership style that fosters teamwork with the team members, and which effectively uses conflict resolution, problem-solving, and delegation skills. 26. Participates in the evaluation(s) of own performance by identifying strengths and weaknesses and by developing a plan to improve areas of weakness. 27. Participates with IDT members in the identification and discussion of ethical conflicts that arise in caring for terminal patients and their families. 28. Occasionally attend Grief Support Groups and Bereavement functions. 29. Accepts other duties consistent with Hospice of San Joaquin's programs and goals.
JOB QUALIFICATIONS 1.
Hold an active LCSW license for a minimum of 2 years in the last 5 years from the Board of Behavioral Sciences, with current practice in social work for the last 5 years. 2. Skills in decision making, program planning/development, and administration. 3. Ability to supervise, train, and evaluate employees. 4. Skill to organize and prioritize activities of self, as well as the activities of staff.
OTHER QUALIFICATIONS 1.
Visual/hearing ability sufficient to comprehend written/verbal information. 2. Ability to sit for lengthy interviews with patients/families. 3. Physically able to maneuver throughout the community. 4. Able to drive or have transportation available. 5. If driving your own vehicle, must have a valid license and adequate insurance
