We are nationally recognized for our first-rate reputation and for delivering outstanding post-hospital rehabilitation and long-term nursing care. We are currently seeking a Full Time (FT) Social Worker to be part of our fun dynamic team!
Position Requirements:
Bachelor's degree in Social Work or Human Services 2-year experience in a health care facility Experience in discharge planning Experience in care planning
Highlight of Responsibilities:
Performs periodic assessments of all residents to determine status and needs. Handles resident concerns and assists residents in a respectful and non-judgmental manner, fostering independence and self-determination whenever possible. Participates in resident care conferences to assist in the implementation of care plans and assists with coordinating family council. Assists residents and families with adjustment that follow initial placement. Encourages resident participation in facility activities. Identifies and documents changes in behavior or personality. Advise the appropriate personnel. Meets with residents and/or families to complete all admissions paperwork. Verifies that all paperwork is in order. Participates in discharge planning meeting and communicates with residents/responsible party discharge plan as well as sets up proper discharge. Performs assesments on new residents to obtain history, establish specific goals and to complete the appropriate section of the Minimun Data Set (MDS). Writes progress notes accoreding to established schedules.
