Job Description

Job Summary We are seeking an energetic and dedicated Social Services Director to lead the long-term care and skilled nursing facility's social services department. In this pivotal role, you will oversee the development and implementation of comprehensive social programs that enhance residents' quality of life, support their emotional well-being, and foster positive relationships with families and staff. Your leadership will ensure that residents receive compassionate, personalized care while maintaining compliance with all regulatory standards. This position offers an exciting opportunity to make a meaningful impact in a dynamic healthcare environment by managing a team of social service professionals and coordinating essential services. Responsibilities Lead and supervise the social services team to deliver exceptional support to residents, families, and staff. Develop, implement, and evaluate social programs tailored to meet the diverse needs of residents, including those with developmental disabilities or behavioral health challenges. Foster strong relationships with residents' families, healthcare providers, community agencies, and internal departments to ensure coordinated care. Oversee case management activities, including assessments, care planning, and ongoing support for residents' emotional and social needs. Manage departmental budgets, ensure compliance with state and federal regulations, and maintain accurate documentation of all social services activities. Provide training and mentorship to staff on best practices in social work, behavioral health interventions, and relationship management. Advocate for residents' rights and facilitate access to community resources, mental health services, and behavioral health programs. Requirements Proven management experience in a healthcare or long-term care setting with supervisory responsibilities. Strong background in program development, especially within long-term care or skilled nursing facilities. Experience working with individuals with developmental disabilities or behavioral health needs. Excellent relationship management skills to build trust with residents, families, staff, and external partners. Demonstrated administrative expertise in budgeting, documentation, compliance standards, and program oversight. Background in social work or related fields with a thorough understanding of social services principles within healthcare environments. Supervisory experience leading teams through change management and quality improvement initiatives.
