Job Description

DPI Staffing is seeking a compassionate and dedicated Health/Human Service Case Manager to join our team in Milford, DE. This role is essential in providing personalized support and advocacy to individuals facing personal, social, health, and economic challenges. ! Milford, DE Schedule:
Full Time 8:00 am to 4:30 pm
Pay Rate:
$22.00 an hour Primary Responsibilities Conduct in-home and community-based investigations related to allegations of abuse, neglect, and financial exploitation Interview clients, caregivers, family members, and other involved parties Assess safety, risk factors, and client needs Document findings thoroughly and accurately using electronic systems Collaborate with community partners, service providers, and internal teams Recommend and coordinate appropriate services or interventions Maintain compliance with state policies, procedures, and ethical standards Target Population Adults 60 years of age and older, and Adults 18 years and older with a disability Qualifications Master's degree in social work or a related human services field (or ability to perform at a master's-level capacity) Prior community-based or human services experience preferred Clean driving record and ability to travel locally for field visits Strong documentation and organizational skills Comfort using computers and electronic case management systems Excellent communication, assessment, and interpersonal skills Ability to work independently while managing a caseload What Is Offered Comprehensive job-specific training All required technology and field supplies provided Consistent daytime schedule Meaningful work serving vulnerable adults Supportive team environment within a state agency If you are committed to making a meaningful difference in people's lives through responsive and compassionate case management, we invite you to apply today!
