Director of Social Services A reputable Skilled Nursing and Rehabilitation Center is seeking a compassionate, experienced, and dedicated Director of Social Services to join our leadership team. This role is essential in ensuring our residents receive high-quality psychosocial support, discharge planning, and advocacy services in a warm and person-centered environment. Position Summary The Director of Social Services is responsible for overseeing the Social Services Department, ensuring residents' emotional, social, and psychological needs are met. This individual will lead discharge planning efforts, coordinate with interdisciplinary teams, and ensure compliance with all federal, state, and facility regulations. Key Responsibilities Oversee and lead the Social Services Department in accordance with SNF regulations and facility policies Conduct comprehensive psychosocial assessments for residents Develop and implement individualized care plans addressing emotional and social needs Coordinate discharge planning, including safe transitions to home or other care settings Serve as an advocate for residents and families, ensuring their needs and concerns are addressed Collaborate with Nursing, Therapy, Admissions, and Case Management teams Ensure compliance with CMS, state regulations, and survey readiness standards Maintain accurate and timely documentation in resident records Facilitate resident and family meetings as needed Provide counseling, support, and crisis intervention when appropriate Participate in interdisciplinary care plan meetings and quality improvement initiatives Supervise and support Social Services staff, if applicable Qualifications Bachelor's degree in Social Work, Human Services, Psychology, or related field required (Master's preferred) Prior experience in a Skilled Nursing Facility or Long-Term Care setting strongly preferred Knowledge of CMS regulations and discharge planning requirements Strong communication, leadership, and interpersonal skills Ability to manage multiple priorities in a fast-paced healthcare environment Compassionate, resident-centered approach to care Benefits Competitive salary (based on experience) Medical, Dental, and Vision Insurance Paid Time Off (PTO) 401(k) Retirement Plan Paid Holidays Career advancement opportunities Supportive leadership team Employee recognition programs Why Join Us?
