Full Job Description Social Services Director Job Summary:

Terraces of Lake Worth Care Center, a Skilled Nursing Facility located in Lake Worth, FL, is seeking a highly motivated and experienced Social Services Director to join our team! As a Social Services Director, you will play a vital role in ensuring the highest quality of care for our residents by monitoring preadmission to discharge practices and coordinating plans for discharges involving community resources and alternate levels of care.

Responsibilities:

Promote transitions of care across the continuum Work with community resources to ensure patients have a safe plan when transitioning out of the facility Possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services Monitor preadmission to discharge practices to assure quality patient care and compliance with regulatory requirements

Requirements:

Minimum of two (2) years of social work experience in a skilled healthcare setting working directly with individuals Strong understanding of community resources and the ability to coordinate plans for discharges Excellent communication and interpersonal skills
